1. Interpretation

1.1. In terms and conditions the following words shall have the following meaning:
1.1.1. "Website" means only and official websites owned by Dental Travel Poland which are www.dentaltravelpoland.co.uk and www.dentaltravelpoland.eu
1.1.2. "We", "Us" or "DTP" means Dental Travel Poland.
1.1.3. "You" means any person accessing the website.
1.1.4. "Costumer" means any person who have made a reservation and required payments via our website.

2. The General Statements. The binding of the agreement.

2.1. Every Customer signifies his/her assent to these "Terms and conditions of Dental Travel Poland" while making a reservation.
2.2 The agreement between DTP and the customer is concluded upon making the reservation.
2.3.These terms and conditions are a part of the agreement and the customer is bound by their content as of the conclusion of the agreement

3. Services

3.1. Dental Travel Poland performs the following services for its customers:
3.1.1. Making accommodation arrangements.
3.1.2. Making dental arrangements.
3.1.3. Providing transportation between the airport and the place of stay and for each of dental appointment in the clinic.
3.1.4. Providing care of english speaking quide and access to 24h customer service help line during the stay
3.1.5. Any other services required by our customers (to an acceptable extent).

3.2. All dental services are provided by dental clinics which take full responsibility for any treatment performed and its consequences. DTP is not responsible for any damage to the customer resulting from the performance of any dental services.

4. Payment Terms

4.1. Types of payment:
a) "Administration fee" covers services pointed in points 3.1.1., 3.1.2, and other administration work. It is an obligatory fee charged while making a reservation. It does not depend on number of patients nor days of stay
b) "DTP fee" covers services pointed in points 3.1.3., 3.1.4. and 3.1.5. It depends on the number of patients and the number of days of stay.
c) Individual payments based on negotiated terms can be prepared for DTP customers if needed

4.2. Payments sequence:
a) Administration fee, DTP fee and accomodation fee - payments are made on-line via website while making reservation (money transfer or credit card).
b) dental services - payments are made in the clinic after visit (credit card or cash)

4.3. Current offer and pricelist of Dental Travel Poland are available at www.dentaltravelpoland.co.uk and www.dentaltravelpoland.eu

4.4. All dental services prices presented by DTP or within dental clinics' pricelists are estimates only: patients will be quoted exact prices of their own treatment plan (which may be higher or lower according to the treatment requires)

5. Customer's obligations

5.1. The Customer shall provide the information about required treatment before making a reservation.

5.2. The Customer shall consult with DTP arrival and departure dates before making a reservation as every visit to dental clinic needs to be planned in advance.

5.3. The Customer shall provide the results of medical examinations (tooth X-ray etc.) performed at the recommendation of Dental Travel Poland. In the event such medical data is not provided, is incomplete, or is provided after the specified deadline, the responsibility and consequences resulting there from shall be with the Customer.

5.4. The Customer is obliged to abide by scheduled dental appointments.

6. Privacy Policy

6.1. Dental Travel Poland respects the privacy of every individual who visits our official website or contacts us by any other means.
6.2. Dental Travel Poland collects personally identifying information when you complete a reservation form or send an e-mail via the site or externally.
6.3. Dental Travel Poland will not sell or make accessible your personally identifiable information to anyone.
